Refused to provide Bank statement saying to fill up the form

Maharashtra Gramin Bank Branch manager Taroda bk Nanded Mr Dilip Patil refused to provide me my own Bank account statement saying that I've to fill a form to get bank statement which was never asked before.
I have a complaint about how a bank can ask to sign and fill up the form just to get a Bank statement of last financial year?
It is my account and I am asking my own account statement still you want me to fill up a form? This is unnecessary documentation. Tell me what is the requirement of such forms. Is there any directive from RBI. If yes send me a copy of the same otherwise I'm going to complaint to higher authorities.

436rupees debited from my account what is the reason I want to last year also 450you debited what money is not comes from trees my family for hard work for my education I leave far from them you are Debiteing all time.
