Obtaining Italian Police Clearance Certificate from New Delhi

MOST URGENT
I urgently need to apply for Italian Police Clearance Certificate for Canadian Immigration purposes. I was in Italy as an MBA student of SDA Bocconi in Milan from 2012 to 2013.
I need your help and guidance with regard to the following.
I understand that foreign national have to apply to the office in Rome, if from applying from outside Italy.
1. Could I please have all the info about the specific form, 2. Address of the office where I need to apply, 3. the documents I need to attach,
4. the fee amount as well as the break-up of different payments and purposes, 5. the value of stamps on the self addressed envelope, 6. once I get the PCC, who are the authorised translators from Italian to English enlisted with the Italian Embassy.
I would be most grateful for an urgent reply to my queries.
