Respected Sir,
                       This is to inform you that I David Smith student of Bachelor of Business administration (BBA) II year, Id no - BBA12a11 will not be able to attend college for 2 weeks from 2nd of march to 17th march, due to suffering from high fever from last three days and as I consulted with a doctor yesterday he said that I am suffering from jaundice and I need to take rest for two weeks. I have also attached the medical report with my application.

              So, Kindly grant my leave application for 2 weeks from 2nd of March to 17th March, 2017. And if I will be able to attend college earlier or want to extend my leave, I will inform you as soon as possible.
