Dear Student,

You have completed first year of study in B.Sc. Nursing (Post Basic) Programme. Now, we would like to offer you the following three courses for the second year. Please fill-in the attached Re- Registration Form, irrespective of whether you have appeared for the First Year Examination or not and whether you have passed or not.
You must register and complete the theory and practicals of all the courses for B.Sc. Nursing Post Basic Programme, simultaneously.

All courses are compulsory.

3. The last date for submitting the Re-Registration Form with a late fee as per schedule :

01.11.2013 to 30.11.2013 - Rs.500/-
01.12.2013 to 20.12.2013 - Rs.1000/-

4. Forms received after the due date and without programme fee shall be summarily rejected. The University is not responsible for postal delays.

IMPORTANT NOTES

(1) Please write your name, Programme Code and Enrolment Number on the back of the Bank Draft to ensure proper credit to your fee account.

(2) Programme fee Rs 15,000/- should be paid in lump sum by means of a Bank Draft obtained from any of the scheduled banks in the name of IGNOU and should be payable at the city where your Regional Centre is situated.

(3) The envelope containing the Re-Registration Form should be superscribed with the words "RE-REGISTRATION FORM B.Sc Nursing (Post Basic) II Year - January, 2014."

(4) Change/Correction of Address and Programme Study Centre: There is a printed card for change/correction of address and change of Study Centre which is available in the Programme Guide. In case there is any correction/change in the address, the learners are advised to make use of proforma provided in the Programme Guide and send it to the Regional Director concerned who will forward the request after verifying the student’s signature to Registrar (SRD), Maidan Garhi, New Delhi-110068. Students are advised not to write letters to any other officer of the University in this regard. Normally, it takes 4-6 weeks to effect the change. Therefore, the students are advised to make their own arrangements to collect the mail from the old address during this period. However, the allotment of the new study centre is subject to the availability of seats for the programme at the new Programme study centre, asked for.

(5) Non-receipt of materials: If you do not receive the study material and current assignments, please contact the concerned Regional Centre.

(6) For any other problem, please write to the Regional Director.

(7) Identity Cards are issued by the Regional Centre. In case of loss, the student may get duplicate identity card on payment of Rs.200/- by means of demand draft, drawn on IGNOU and payable at the place of Regional Centre.

(8) Change of Address and Study Centre is not permitted until the commencement of the session.
